Oświadczenie podmiotu powierzającego wykonywanie pracy cudzoziemcowi dotyczące okoliczności, o których mowa w art. 88j ust. 1 pkt 3-7 ustawy z dnia 20 kwietnia 2004 r. o promocji zatrudnienia i instytucjach rynku pracy
ma być własnoręcznie podpisane przez właściwego mocodawcę
(tj. właściciela firmy lub osobę uprawnioną do reprezentacji zgodnie z KRS).

Niedopuszczalnym jest złożenie podpisu przez występującego w sprawie pełnomocnika.

Declaration of the entity entrusting the performance of work to a foreigner regarding the circumstances referred to in art. 88j paragraph 1 points 3-7 of the Act of 20 April 2004 on employment promotion and labor market institutions
are to be signed by the appropriate principal
(i.e. the owner of the company or a person authorized to represent in accordance with the National Court Register).

It is unacceptable for a representative to sign the case.

Декларація суб’єкта господарювання, який доручає виконувати роботу іноземцю, щодо обставин, зазначених у ст. Абзац 88j 1 пункт 3–7 Закону від 20 квітня 2004 р. Про сприяння зайнятості та інститутам ринку праці
повинен бути підписаний відповідним довірителем
(тобто власником компанії або особою, уповноваженою представляти відповідно до Реєстру національних судів).

Неприпустимо, щоб представник підписував справу.
